- We validate every authorization and subpoena, date-stamp it, prepare the custodian affidavit and release the records.

Common questions
What do I need to request my records?
A signed authorization and a photo of your driver's license or other government photo ID. The chat walks you through both, and you sign the authorization online.
What does it cost?
$15.00 flat for processing and handling, with records sent electronically to your email. A mailed paper copy is $45.00 for processing and mailing, plus $0.50 per page for printing. Records are released after payment is received.
How will I receive my records?
Electronically, by secure link sent to the email you provide. If you need a paper copy, choose mail in the chat.
The patient is my child or has died. Can I get the records?
Yes, if you are the patient's parent, guardian, or the executor or administrator of the estate. Answer "No" when the chat asks whether these are your own records, and upload proof of your authority, such as a birth certificate, guardianship order or letters testamentary.
The chat says you don't have my doctor's records.
Check the spelling of the doctor's name and try again. If it still isn't found, email support@squeezedocs.com and we'll check for you.
How long does it take?
Complete, paid requests are processed within the time limits set by law, and subpoenas by their production date.
Is my information secure?
Your request and documents are sent over an encrypted connection and kept in our HIPAA-compliant records system. Every access and release is logged.
Can I cancel my authorization?
Yes. Write to support@squeezedocs.com or SqueezeDocs, Inc., Custodian of Records, 23505 Crenshaw Blvd., Suite 236, Torrance, CA 90505. A cancellation doesn't undo releases made before we received it.
